About the Role
You'll take on a patient caseload from day one, delivering high-quality, personalized 1:1 (and occasionally live group) coaching that drives sustainable behavioural change aligned with clinical plans. You'll proactively engage patients based on clinical needs and coaching judgment, adapt coaching interventions, and test new approaches to improve retention and satisfaction. You'll respond to patient queries within agreed SLAs, maintain accurate documentation, mentor new starters, and monitor your own performance data to adjust your coaching approach. You'll participate in clinical audits, monitor patient safety risks, and escalate concerns as needed. You'll also identify opportunities to introduce relevant Juniper services aligned with patient goals and collaborate cross-functionally with teams like the shop, community, or PES to support the broader patient care journey.
